Crynodeb
You will need to work as part of a wider team but also be able to work autonomously, prioritising your workload and communicating delays and concerns in a timely manner. Work as part of the multidisciplinary ward team, gaining an insight and knowledge onto the needs of all the patients on their allocated wards. Support patients, their families and carers to ensure that assessments and discharge plans are fully understood and agreed. Attend the daily board meetings to proactively identify patients who are ready for discharge and transfer to a backend medical bed.
