Crynodeb
The post holder will work both directly and indirectly with patients and their carers, using a range of IT systems to assist in coordinating patient care throughout their hospital journey. The post holder will coordinate appropriate services to maximise the delivery of streamlined diagnostic pathways, thereby improving quality of care and experience for patients and their carers. The post holder will work across management teams and organisational boundaries, whilst linking in with the Cancer Centre, and contribute to plans and developments. The post holder will be responsible for the review and updating of the Lower GI tracking spreadsheet which informs the cancer pathway. They will also be responsible for ensuring that any required actions take place.
