To work as part of the Bridport Community Mental Health Team. To be responsible and professionally accountable for a patient caseload within the community. The postholder will be working with people with a serious mental illness, providing high quality nursing care and treatment alongside the multi-disciplinary team.

Bridport Community Mental Health Team is a well established multidisciplinary team made up of highly skilled mental health professionals. The team have a compassionate and proactive approach to patient care.

The CMHT has strong links with the local community and are at the forefront of an increased number of joint working initiatives with primary care and the voluntary sector.

This is an exciting time for Community Mental Health Teams across Dorset Healthcare due to the implementation of the transformation plan.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

The postholder will be responsible for a caseload in the community , undertaking bio-psychosocial assessments, cover the duty rota and providing evidence-based interventions with their patients.

If the postholder is a registered nurse, they will be rota'd on to the Depot clinic and also support the nursing team with taking bloods and be part of managing the Clozaril Clinic.
