Key Responsibilities:
Effect safe hospital discharges for service users using the Discharge to Assess (D2A) model.
Undertake assessments and discharge planning in a fast-paced hospital environment.
Work closely with hospital teams at the Royal London and neighbouring hospitals to ensure timely and safe discharge arrangements.
Transfer cases to locality teams upon safe discharge—no waiting list or long-term case holding.
Contribute to a 7-day service operating Monday to Sunday from 9am to 8pm.
Work as part of a supportive team with a strong management structure and a hybrid model of on-site and home working.
Deliver high-quality social work practice, ensuring legal and policy compliance at all times.
Essential Requirements:
Degree/Masters in Social Work and SWE registration.
2+ years of post-qualification experience.
Strong knowledge of UK social care legislation and practice including Care Act, MCA, and Safeguarding.
Experience in hospital discharge and D2A pathways.
Ability to work flexible hours (Thursday 3pm–8pm, Friday 9am–8pm, Saturday 9am–8pm, Sunday 9am–8pm).
Right to work in UK.
