Crynodeb
The role will primarily be based between the Emergency Department and the Emergency Floor and eventually within a specific frailty SDEC footprint on either site. The role will be working with our Acute Frailty Team but will also involve the day to day care of patients living with frailty on the inpatient wards. We are committed to enhancing the service we provide to patients living with frailty in West Sussex, across both the Worthing and St Richards sites. Our service development plans include building on our already established Acute Frailty service, by developing a frailty SDEC, and expanding our rapid access clinics. We aim to take a system based approach to providing excellent patient care to the frail population and work with our community partners and key stakeholders to establish effective admission avoidance pathways with initiatives such as Virtual Ward as well as community geriatrics. The Trust has recent merged and with that comes further opportunities. As a teaching hospital, and with links to the medical school, we encourage our team to share their skills and experience with the junior teams and as a result, we have a close team who support one another. As an experienced registrar, you will be expected to lead the team of junior doctors, supported by the Consultants, in providing high quality care and ensuring timely discharge for inpatients across the Emergency Floor and across the DOME Wards.
