Crynodeb
Job Responsibilities Manage pharmacist-led clinics for diabetes and hypertension. Undertake SMRs for high risk or complex patients and those with multiple comorbidities. Reconcile medicines across transitions of care, ensuring safe and consistent prescribing. Implement and monitor adherence to MHRA drug alerts, NICE recommendations and local policies. Lead or contribute to prescribing audits, QOF-related work and PCN DES projects. Provide clinical input into repeat-prescribing systems and safe-prescribing workflows. Deliver education and training for practice and PCN staff on therapeutics and medicines management. Participate in multidisciplinary team meetings and contribute to shared clinical decisions. Support development of local prescribing guidelines and quality standards.
