To maintain excellent communication with patients, carers, colleagues and all members of the MDT. To liaise with members of the multidisciplinary team regarding the care and treatment of patients to optimise health outcomes. Work in conjunction with the Clinical Nurse Specialist and ward nursing team in assessing the priorities of patient care and the staffing requirements. To acknowledge the limits of your professional competence and only undertake practice and accept responsibilities for those activities in which you are competent. To maintain accountability for assessing, planning, implementing and evaluating programmes of care within the framework of team/primary nursing incorporating the Named Nurse initiative. To promote a patient focused multidisciplinary approach to care in collaboration with carers, health care professionals and other agencies. Help patients gain access to health and social care, information and support relevant to their needs. To educate patients and their carers as required where a need has been identified. To provide information that enables patients to make choices about adopting a healthier lifestyle. To complete patient documentation correctly, appropriately and in a timely manner. To maintain a safe clinical environment and work with other members of the team to produce health care environments that are conducive to safe, therapeutic and ethical practice. To assist in establishing and monitoring protocols/care pathways. To refer and signpost to fellow professionals as needed. To respect and maintain patient confidentiality and privacy. To respect the cultural and ethnic backgrounds of all patients. To acknowledge the importance of support for self and accessing personal clinical supervision. Performance To be fully aware of national service requirements and targets and to ensure care is provided in line with agreed National CHD competencies, Trust ICPs and the core principles of the Nursing Strategy To familiarise and continuously update yourself with Statutory, Trust and local Clinical policies, procedures and protocols. To facilitate a high quality specialist service through monitoring and audit and the maintenance of other quality initiatives agreed by the Trust. To ensure the active inclusion of the views of patients and carers in the monitoring and development of service provision through patient groups and open days. To utilise information systems to aid regular audit of the clinical role and to provide an annual activity analysis and specialty report. To identify and intervene proactively where circumstances contribute to an unsafe environment for patients and staff. To provide a role model, demonstrating knowledge and high standards of clinical practice. To maintain accurate records of the clinical service while ensuring that confidentiality of information is adhered to. To actively participate in relevant meetings both within and outside the Trust, to ensure an awareness of national and local guidance, directives and developments To participate with other team members in service planning and the service review process as appropriate To follow Trust procedures reporting incidents and notification of clinical risk incidents related to patients, staff or visitors. To act in accordance with the Nursing and Midwifery Council Code of Professional Practice for Nurses/Midwives and Health Visitors and the Scope of Professional Practice. To be accountable for his/her practice and to take every reasonable opportunity to sustain and improve his/her professional competence. To adhere to Trust policies and service guidelines and protocols. Workforce To contribute to the education and training of appropriate health professionals and in particular to develop nurses skills with regard to the care of those with skin cancer. To act as a role model to staff. To contribute to the development of resource and teaching packs for both patients and staff. To recognise own need for continual education and skill updating in cancer treatment and care and to take measures to meet these needs. To maintain own professional development through annual appraisal of clinical performance and the production of a professional development plan. To complete mandatory training as determined by the Trust. Financial As part of the team, take responsibility for the effective and efficient use of resources, being aware of resource requirements and limitations. Ensure all equipment and supplies in the clinical area are maintained to enable delivery of efficient and safe care. As part of the team, evaluate equipment and make recommendations relevant to purchasing policies in conjunction with Procurement. To help identify requirements to carry out the service and assist the manager in the implementation and management of the service provided. To be aware of the Budgetary requirements and support the manager in working within this Partnerships Internal Key Working Relationships: Nursing Team Members of the MDT Practice Development Nurse/Clinical Skills Facilitator Specialist Cardiac Nurses Senior Nurses, Matrons and Heads of Nursing within the Trust Medical Team Radiologists / Physiologists General Maintain effective communication with patients, carers and professionals to ensure seamless service delivery. Contribute to the review of patient and family care as part of the multidisciplinary team. Develop good working relationships with staff and colleagues in all settings. Act as the patients advocate, offering education on all available treatment options to enable them to make informed choices, negotiating with the medical team to agree a pathway of care. Ensure patients and their families are given appropriate verbal and written information regarding the disease process and planned care. Provide continuity of care for patients throughout their care journey, supporting them and their relatives and carers through treatment and beyond. Educational: To identify areas for own learning and development and develop teaching and training skills. With support, contribute to the design and provision of up to date training programmes for a variety of health care professionals. Identify, assess and meet the educational needs of the patient and the family Audit and research Participate in the collection and evaluation of clinical audit within, and when appropriate beyond the speciality. Contribute to the directorate and MDT audit and research programmes as requested. Identify topics and highlight areas for nursing audit and research in relation to specialist area of practice.
