MAIN DUTIES AND RESPONSIBILITIES Support the 0-19 Service Team to achieve Asthma Friendly Accreditation in Enfield Schools, maintain and update this Accreditation. To work in partnership with the 0-19 School Nursing and Health Visiting Teams, service users and all stake holders in promoting the health and well-being of Children, Young People aged 5-19 years and their families. This will be achieved through effective delegation, workload management, monitoring of care standards, leadership. To work in partnership with team members, service users and all stake holders in promoting the health and well-being of Children, Young People aged 5-19 years and their families in the school environment. This will be achieved through effective evidenced based practice, NICE Guidelines, Government Directives, NHS Plans, Specialist Nurses, GPs and other Specialist Practitioners, workload management, monitoring of care standards. Identify and monitor health needs of a given population using profiling tools and facilitate the implementation of evidence-based care. The practitioner will work in line with the Healthy Child Programme and all policies that promote and safeguard children 5-19 years and their families. To manage and prioritise workload according to the identified health projects needs within a defined caseload ensuring the effective use of resources, keeping accurate data and measuring success of work via audits and Clinical Governance. The main link with schools/family hubs to promote, advise and guide current best practice/NICE guidelines and best practice intaitives eg Asthma Friendly Schools;Sickle Cell guidance; Anaphylaxsis best practice; NICE guidelines; current evidence based practice.1.7 An annual work plan for long term conditions will be agreed between the Service Lead for School Nursing and the School Nurse for Long Term Conditions to inform the workstreams for the year ahead. Priority workstreams will be identified based on local need, gaps in provision and required service developments.These priority areas will be agreed in partnership with Enfield Councils Public Health Team at the Operational Management Team Meeting. RESPONSIBILITY FOR PATIENTS AND SCHOOLS/FAMILY HUBS/LOCAL AUTHORITY/NHS ORGANISATIONS. 2.1 To keep contemporaneous notes and accurate records and maintain client/school/family hub confidentiality according to NMC and Trust guidelines. 2.2 To work within the policies and guidelines set out by the Trust and NMC- Code of Professional Conduct. 2.3 To adhere to all Trust policies, procedural guidelines and current legislation. 2.4 Ensure up to date with all internal training and development. 2.5 Regular requirement to write reports and collect current data regarding progress and changes made in the community on a monthly basis. Also speak and communicate with service users in a caring professional manner. RESPONSIBILITY FOR POLICY AND SERVICE DEVELOPMENT 3.1 To undertake such other duties as may be reasonably required from time to time as are consistent with the responsibilities of the post. 3.2 To participate in research/audit and statistical data which are approved and agreed. 3.3 To contribute to policies and the development and implementation of care pathways. 3.4 To participate and contribute to developing practices and protocols 3.5 Deliver evidence-based practice to ensure high standards of care and contribute to audits and quality improvement projects. RESPONSIBILITY FOR FINANCIAL AND PHYSICAL RESOURCES 4.1 To ensure the correct use and storage of resources. Systems and Equipment To maintain equipment and report to senior staff when there is any malfunction To be aware of Health and Safety hazards in the working environment and report them to senior member of staff. RESPONSIBILITY FOR LEADING AND MANAGING 5.1 To be responsible for the supervision and delegation of duties of support staff. 5.2 Frequent concentration to complete assessment and analysis of client and staff needs. Work pattern altering to meet service need and prioritising work issues accordingly with changes faced 5.3 Day to day supervision of staff/local induction and line management responsibility of junior staff ensuring they have support; leadership and professional guidance. 5.4 Car driving for liaison with agencies and to attend meetings and training 5.5 A team player who is able to share and reflect nursing expertise and current best practice. RESPONSIBILITY FOR INFORMATION RESOURCES 6.1 To have excellent computer skills and be willing to undertake Information Technology Training to develop skills in electronic record keeping and data collection. 6.2 Ensure up to date with all internal training and development. 6.3 Regular requirement to write reports for case conferences and other meetings as appropriate. 6.4 Responsible for the maintenance of own record keeping on patient record information system (RIO). 6.5 To have excellent computer skills and be willing to undertake information Technology training to develop skills in electronic record keeping and data collection. RESPONSIBILITY FOR RESEARCH AND DEVELOPMENT 7.1 To participate in research/audit and statistical data which are approved and agreed. 7.3 To contribute to the planning and implementing of effective health promotion and health education programmes for client group within communities to support Public Health Outcomes. 7.4 Ensure up to date with NICE guidance and how to implement into day-to-day clinical practice.
