Communication To have a basic understanding of the psychological and spiritual needs of patients and their families and communicate with them as appropriate, but referring to the Clinical Nurse Specialist or District Nurse as necessary Establishes and maintains effective communications with patients, carers and health professionals in a professional manner In line with UHCW documentation policy update patient records ensuring entries are accurate relevant legible and timely and communicates care provided appropriately.
