The Tower Hamlets Stepney and Wapping Neighbourhood Mental Health Team is a well-established team within East London Foundation Trust and operates as an integrated multi-disciplinary team with staff from both the East London NHS Foundation Trust and the London Borough of Tower Hamlets working together to provide a high quality service for people with severe and enduring mental health needs this is inline with Transformation Project. The team actively works in partnership with service users, carers, the Local Authority and with other statutory services and local voluntary groups.

The post holder will hold a caseload and work within the principles of the agreed model of care for the service as laid down in the operational policy. The post holder will work as an autonomous practitioner within an integrated team. This will involve the assessment planning, icare act assessment, mplementation and evaluation of the care provided and will liaise with all agencies involved with the client. The post holder will be required to administer prescribed medication which includes intramuscular injections.



Develop and maintain a high standard of practice based on assessment through the use of psycho education, the Recovery Model and supportive clinical management.

Liaise and establish good working relationships with referring agents, colleagues in Primary Care services, Adult social care Services, Other Neighbourhood Teams, In-Patient services, Criminal Justice, Voluntary Sector and other agencies as appropriate.

Ability to work autonomously and within MDT setting.

To work with the multi-disciplinary team in developing plans that offer supportive crisis care in the least restrictive way.

Ensure compliance with the requirements Safeguarding Adults and Children’s policies. T

Recognize the role of advocacy, promote concept of autonomy and self-empowerment and ensure members of the team comply with this.

Have an understanding of the KPI’s for the service an participate in improving team quality and performance.

Identify own educational needs ensuring professional updating and development as appropriate.

Ensure full potential through supervision, appraisals and performance management.

Ensure that you are suitably skilled in addressing the physical health needs of service users.
