We are Unique Support & Care Limited, a care provider and recruitment agency.
We are currently recruiting for a care coordinator on behalf of a well-established charity committed to delivering exceptional care and support services.
Title of Job: Care Co-ordinator
Hours of Work: 35 hours per week 9:00 to 17:00 Monday to Friday
Reports to: Homecare Registered Manager
Main Purpose and Scope of the Job
The Care Coordinator is the first point of contact for carers, customers and agencies. They will liaise with Camden’s Adult Social Care, NHS Services and care brokers to get referrals. They will notify of changes in care services and ensure all information is up to date. They will make sure that all customer care packages are covered and manage any day-to-day changes in care packages.
Main Tasks and Responsibilities
• Accepting allocating and processing referrals for new work promptly
• Accepting and processing changes and amendments to existing care packages
• Covering and processing all short-term changes to care packages
• Assigning care workers to customers, whilst working within the standards and requirements of the charity
• To ensure all visits are delivered according to contract
• To check ECM to ensure all visits are delivered on time and avoid missed visits
• To ensure adequate covers are provided to clients during annual and sickness leave
• Talking to clients about their care and their packages
• Keeping records and paper files up to date with changing information
• To book training and workshops for care workers to develop their skills
• To liaise with Social Services and Homecare Commission along with other similar bodies that relate to the service
• To ensure care workers and clients hours are up to date on WebRoster and ready on time for invoices and salary payment
• To provide carer workers with the necessary equipment (such as gloves, apron, and shoe cover) required for their home visit and place order with the supplier in time for the equipment
• To ensure all visits and real time are amended on WebRoster on time for the Finance Team to raise invoices and salary payment for care workers
• To ensure care workers’ annual leave are taken within the financial year following the charity’s policy
• Do regular checks to ensure that all carer workers and clients’ personal data is updated
• To provide and support the Head of Homecare and the Registered Manager with weekly KPI and other reports
• To arrange quarterly supervision for care workers
• Working with other members of the team to ensure high quality service provision
• Cover on-call coordination in emergency situations
General Responsibilities
• Preparation of reports as required
• General administration duties, filing, writing up reports and taking messages and dealing with homecare enquiries
• To keep accurate records adhering to relevant policies regarding confidentiality and information sharing and equal opportunities
• Contribute to the sharing of good practice and support continuous improvement throughout the charity
• Participate in supervision and appraisal with their line manager. To work with the line manager to review professional development and undertake training as agreed
• Contribute through team meetings, training and outreach/events to the development of the charity’s services
• Any others duties are considered commensurate with the post
PERSON SPECIFICATION
Essential Qualifications
• NVQ Level 3 in Social Care
• A willingness to attend further training as part of the role
Essential Experience
• Experience of Microsoft Office packages including Outlook, Word and Excel
• Experience of accurate record keeping and filing, both electronic and paper.
Essential Skills & Abilities
• Good planning and organisational skills and the ability coordinate the work of carers
• Ability to give clear and understandable instructions
• Ability to work in a highly pressurised and busy office environment and stay calm under pressure
• Ability to communicate effectively with customers, professionals and colleagues by email, telephone and face to face
• Proven numeracy skills, to be able to carry out basic financial responsibilities
• Ability to maintain a high degree of confidentiality in relation to a range of information
• Ability to communicate in a professional and effective manner
