Support scheduling of liver surveillance clinics and patient assessments. Track patient engagement, ensuring timely access to surveillance scans and diagnostics. Maintain and populate the liver surveillance database, ensuring data accuracy and preparing reports for NHS E. Collaborate with healthcare teams to fast-track patients with abnormal results into appropriate care pathways. Provide direct patient support, including appointment reminders and navigation through the healthcare system. Work closely with multidisciplinary teams (MDT) to ensure streamlined patient care across sites. Ensure all patient records are tracked, stored securely, and transferred between trusts as required. Contribute to audit processes, service development, and ongoing improvement of the surveillance pathway. Data Collection and Analysis: Collect and update patient data, ensuring accuracy for reporting and service improvement.
