Assist in holistic assessments and contribute to care planning by providing information, emotional support, and practical advice to patients, families, and carers, under the supervision of registered professionals. Participate in the implementation of evidence-based palliative rehabilitation services, under the guidance of registered therapists and clinicians. Assist in the assessment and delivery of non-pharmacological symptom management interventions in line with agreed local and national practices. Support patients in accessing appropriate health and social care services, helping them understand and navigate available resources to make informed decisions about their care. Communicate sensitively and clearly with patients, families, and professionals using face-to-face, telephone, and electronic methods. Share responsibility for the clinical caseload within the locality, supporting patients with complex needs and ensuring timely and coordinated care. Work both collaboratively within the multidisciplinary team and independently within the boundaries of your role, seeking guidance from senior colleagues when needed. Contribute to the training and support of colleagues and volunteers, sharing knowledge and promoting best practice in palliative care. Support the implementation of locality-based service improvements, contributing to quality initiatives and helping embed efficient and effective care processes. Help build and maintain positive relationships with external partners and stakeholders, promoting the values and reputation of Farleigh Hospice.
