We are looking for an enthusiastic and compassionate Staff Nurse to join our Discharge Lounge teams at both The Royal Shrewsbury Hospital and The Princess Royal Hospital in Telford. This is an exciting opportunity to play a vital role in supporting safe, timely and effective patient discharges.

The Discharge Lounge is a fast paced environment that provides a comfortable area for patients to wait for their medication, discharge letters and transport before leaving the hospital. This role requires a high level of effective communication as you will be liaising with wards, relatives, care agencies, community hospitals, the site team and many others to facilitate discharges and maintain patient flow.

We are looking for someone who is organised, proactive and able to problem solve whilst under pressure.

You will ensure patients are cared for with dignity and respect whilst helping to maintain patient flow across the hospital. The Discharge Lounge is an integral part of the UEC pathway supporting the hospital by creating capacity on wards which allows for flow from our Emergency Department and enables timely ambulance handovers.
• Assess, plan, deliver and evaluate high-quality patient care within the Discharge Lounge.
• Transferring patients from wards to the Discharge Lounge in a timely manner to support capacity and flow.
• Support safe and efficient discharge processes in collaboration with the wider disciplinary team.
• Ensure patient safety is maintained at all times. Record and report adverse and potentially adverse events and assist with investigation of such events, ensuring learning is shared with the wider team(s).
• Record information about patients in a safe, accurate and consistent manner using appropriate tools and techniques, including electronic and written formats as close to point of care as possible.
• Monitor patient well-being while they await transport, medication and discharge letters.
• Provide clear communication and reassurance to patients and relatives regarding discharges plans and delays.
• Assist with documentation, handovers, and coordination of discharge planning, including referrals to other services.
• Ensure a safe and organised environment, maintaining infection control and health and safety standards.
