Daily prescription queries Providing support to Prescription Coordinators and Clinical Pharmacists in managing daily patient prescription requirements. To include the resolution of patient / pharmacy queries. In particular focussing on: Out of stock medications Pharmacy queries ETP queries Discharge Summaries Managing and processing discharge summaries and clinical letters. Undertaking patient facing and patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients. Reviewing Patient Medication Running Blood Pressure and medicines monitoring clinics either patient facing or virtually, including in Care Homes. To include carrying out medicines optimisation tasks including effective medicine administration (e.g. checking inhaler technique), supporting medication reviews and medicines reconciliation. Where required, utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively. Providing specialist expertise, where competent, to address both the public health and social care needs of patients, including lifestyle advice, service information, and help in tackling local health inequalities. Vaccination Programmes Supporting vaccination programmes for patients and staff as required MHRA Alerts Managing MHRA alerts and actions, including leading the receipt and understanding of the alert, communicating with PCN contacts and completing all associated actions. Actions may include, carrying out audits and patient searches, contacting patients and sending patient letters. Eclipse Alerts Managing Eclipse alerts. To include running searches and deciding on the appropriate course of action based on findings, communicating with PCN contacts and completing all associated actions. Actions may include, carrying out audits and patient searches, contacting patients and sending patient letter. Audits Undertaking clinical audits, supporting quality improvement measures and contributing to the Quality and Outcomes Framework and enhanced services. Assisting in the delivery of medicines optimisation incentive schemes (e.g. medicines switches) and patient safety audits. Leadership Being the point of contact for ICB for cost switches and cost savings Supporting initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing. Providing training and support on the legal, safe and secure handling of medicines, including the implementation of the Electronic Prescription Service (EPS). Developing relationships with other pharmacy technicians, pharmacists and members of the multi-disciplinary team to support integration of the pharmacy team across health and social care including primary care, community pharmacy, secondary care and mental health. Support practice reception / admin teams in sorting and streaming general prescription requests, to allow GPs and clinical pharmacists to review the more clinically complex requests. Supporting the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings Taking a central role in the clinical aspects of shared care protocols and liaising with specialist pharmacists for more complex patients. Support trainees such as Pre-registration Pharmacy technicians and Foundation Pharmacists with their learning needs as required All duties are performed within your level of competence. You may be gathering data and information, performing the task to your level of competence, and then liaising with or handing over to another professional. You will be supported to develop and expand your boundaries of competence as you grow in the role. You will need to register with CPPE and complete the 15-month Primary Care Education Pathway (PCPEP) for which you will be fully supported.
