Crynodeb
The role of the Chair is generally confined to the day of the review and the completion of C(E)TR documentation. It is not expected that Chairs will have on-going involvement in cases beyond this. Chairs may be contacted in relation to queries from the CTR review. Ensure that the CTR is carried out in a manner that: o Is based on the person-centred values laid out in the CTR Policy. o Is independent, fair and constructive. o Ensures that all relevant views are heard and discussed. o Ensures that the views and wishes of person whose care and treatment is being reviewed, and their family members if involved, are clearly established and are at the centre of the reviews discussed. The delegated chair must also ensure good communication with the responsible commissioner prior to and following the review. Ensure that the Expert by Experience is able to be an equal and integrated member of the team. Ensure that information is made available in a form that is accessible for all. Establish an outline plan for the review day and, if required, to modify this according to any particular issues that may emerge during the review. To help the team develop a pen-picture at the beginning of the day of the person whose care and treatment is being reviewed. Be alert to any issues of concern regarding the welfare and safety of the person and to respond immediately and appropriately to issues that require prompt or urgent action and / or escalation. To ensure that the team is able to have discussions with the clinicians currently responsible for the persons care and treatment and also those who may be supporting them. To enable the team to positively challenge aspects of the persons care and treatment and plans for future care where necessary. To clarify and summarise the findings and recommendations of the review panel both on the day and in a subsequent report.
