The Role
Cornerstone’s Shared Lives and Short Breaks service provides both transitional and long-term placements to people aged from 16 years old who require care and support due to a disability, health condition or support need. Shared Lives Carers and their families provide support and accommodation in their own homes.
As this specialist, sector-leading service continues to grow throughout the West of Scotland, we are looking for a full time Assessor to join the team. Our Assessors are responsible for assessing, training and supporting carers throughout the full process of their application ensuring they have the right values and understating for a role with Shared Lives.
As a Shared Lives Assessor, you will be responsible for managing and monitoring Shared Lives arrangements, including employing appropriate interventions to support carers and provide direction and guidance to ensure exceptional standards of care and support are being delivered to those individuals in care.
You will also visit our carers regularly at their own homes to offer support, development and monitor the arrangements in place offering coaching and mentoring where needed to promote safe, person-centred support and care to the individuals.

About You
As regular travel will be required throughout Glasgow and the surrounding areas, it is essential that you hold a full, clean UK driving licence and have access to your own vehicle.
To be a great Shared Lives Assessor, you will:
be empathetic with a warm, friendly personality and a positive, can-do attitude
be able to demonstrate exceptional communication skills and patience
be able to think on your feet and use your own initiative
have demonstrable experience in comprehensive assessment and reporting
be able to build and manage good professional relationships
have knowledge of the relevant legislative framework of ‘Adult Placement Services'
have a SVQ Level 4 or extensive experience in health and social care or social work and working with adults who have learning disabilities, Autism or complex needs

Shift Patterns
This a full-time role consisting of 39 hours per week, Monday to Friday. Although this role can be home based, you will be expected to attend our offices for meetings, visit carers and attend events. There will be an element of travel and therefore we are looking for the applicant to live in Glasgow, North Lanarkshire or surrounding areas.
