Cancer Pathway Trackers play a vital role in supporting the timely and effective management of patients referred with suspected cancer. This role involves accurately documenting and monitoring patient progress throughout their cancer pathway to ensure all information is correctly recorded in the Somerset Cancer Register, the Trust’s Cancer Tracking Database.

Key responsibilities include reviewing and interpreting clinical correspondence (such as clinic letters) to determine the appropriate next steps in each patient’s diagnostic or treatment journey. Trackers will also be responsible for transferring essential information from the Electronic Patient Record system (EPIC) into the Somerset Cancer Register, ensuring data is entered accurately and in line with Trust standards.

Cancer Pathway Trackers are responsible for proactively monitoring patients referred under the Urgent Suspected Cancer (Two-Week Wait) pathway. They ensure that all patients are seen, investigated, and diagnosed within the nationally agreed timeframes, and are expected to escalate any delays or concerns promptly and appropriately.

They are to be responsible for alerting and escalating to the Shared Care Navigator Team Leader and Cancer Performance Manager when problems and delays arise.
• To work independently with minimal supervision and as part of a team prioritising activities to ensure workload is completed in a timely manner
• To follow the departmental escalation process by informing the Cancer Services Team Leader if they become aware of any potential issues/breaches against the Cancer Waiting Times targets and update Somerset with all actions taken, contributing towards any potential breach reports.
• To utilise hospital systems, such EPIC and Somerset Cancer Register, to track and monitor patients appropriately through their suspected cancer pathways.
• Accurately record referrals onto Somerset from EPIC, ERS or referral letters. To chase images and relevant reports from other hospitals and upload to EPIC.
