About Prevalent Care Limited:

Prevalent Care is a dedicated family support service with a core mission to bridge the gap between families and the complex legal and social care systems they navigate. We provide essential early intervention services across Dorset, working to safeguard children and empower parents.

Our approach is rooted in being trauma-informed, child-focused, and non-judgmental. We believe in every family's potential and work collaboratively with solicitors, social workers, and local authorities to build resilience and foster safe family connections. As a non-residential service, our focus is on delivering expert support through supervised contact, advocacy, assessments, and tailored parenting programmes.

About the Role

We are seeking a proactive and empathetic Outreach Family Support Worker to provide hands-on, practical support to families in their own homes and communities. You will build trusting relationships and work intensively with parents and children to address specific needs, build on their strengths, and help them navigate challenges. This autonomous role is perfect for a self-starter who is skilled at engagement and passionate about creating positive change.

Key Responsibilities

Conduct home visits to provide tailored support, practical help, and mentoring.
Develop and implement goal-oriented support plans in collaboration with families.
Help families access local services, attend appointments, and engage with schools.
Model positive parenting strategies and communication skills.
Maintain clear records of visits and progress, and report any safeguarding concerns.

What You’ll Need

A minimum of 1 years' experience in a family support, outreach, or social care role.
Proven ability to work independently and build rapport with vulnerable families.
Strong knowledge of local services and resources across the Dorset area.
An Enhanced DBS (or willingness to apply for one) and a full UK driving licence with access to a vehicle.

Highly Desirable

An NVQ Level 3 (or above) in Health and Social Care or a related field.
Experience working with families on Child Protection or Child in Need plans.
