Job Overview:

We are seeking an experienced and motivated Care Coordinator to join our team. This is a key leadership and organisational role that ensures the smooth delivery of care services to our clients. The successful candidate will be responsible for planning, coordinating, and overseeing care delivery while supporting and supervising a team of carers.

Key Responsibilities:
Coordinate and schedule care visits in line with client needs, staff availability, and continuity of care.
Work closely with the Registered Manager and support staff to deliver quality outcomes.
Monitor the effectiveness of care plans and make adjustments as needed.
Conduct care assessments, risk assessments, and reviews in clients’ homes.
Supervise and support care staff, including carrying out spot checks and performance reviews.
Ensure all care delivery is compliant with SSSC standards and the Health and Social Care Standards.
Maintain accurate and up-to-date records using our electronic care planning system.
Respond promptly to changes in care needs, client emergencies, or staff absences.
Participate in the on-call rota as required.
Maintain effective communication with clients, families, social workers, and healthcare professionals.
Assist in recruitment, onboarding, and training of care staff.

Essential Skills & Attributes:

Excellent organisational and time-management skills

Strong leadership and communication abilities

Ability to multitask and work well under pressure

Compassionate, empathetic, and client-focused

High level of IT literacy (Microsoft Office, electronic care systems, rota management tools)

Knowledge of Care Inspectorate regulations and SSSC Codes of Practice

Able to work flexibly, including occasional evenings or weekends as required

Qualifications & Knowledge Required:

SVQ Level 3 in Health & Social Care (or working towards it) – essential

Supervisory or management qualification (or willingness to work towards one) – desirable

Sound understanding of Scottish health and social care legislation and best practices

Knowledge of SSSC registration requirements and responsibilities

Full UK Driving Licence and access to own vehicle – essential

Location Coverage:

You must be willing and able to travel across Moray and North Aberdeenshire, including areas such as Elgin, Keith, Huntly, Banff, and surrounding rural communities.

SSSC & PVG Scheme:
