Responsible for the overall delivery of a range of evidence based interventions to service users based on identified needs utilising agreed patient reported outcome measure tools and assessments such as CAARMS Ensure service users are seen in a timely manner by the right person, at the right time, in the right location. Implement a team approach to case management where practitioners are not isolated in decision making and have the ability to access a range of expertise to inform decision making. To take responsibility for supervising and managing clinical activity and casework Develop robust interagency relationships with key stakeholders and other outside agencies supporting the development of the integrated care pathways and locality working. Ensure the efficient and effective operation of the service, managing budgets, matching resources to need, and coping flexibly with workload pressures. Work collaboratively with the Mid & South Essex care unit corporate support services to establish and manage operational systems which highlight early warning of deviation from financial / activity targets, and initiate corrective action. Assist in planning and managing the implementation of projects to improve facilities and working practices, as agreed with the Associate Director . Ensure through implementing effective performance management that all key performance indicators and targets identified by the Trust are met. Responsible for the coordination and management of the team in providing a safe, effective and therapeutic clinical service, ensuring the delivery of high quality care and support.
