Working across the health and social care landscape the post holder will lead, coordinate and provide forward momentum to the programme of change related to the Transfer of Care Hub, based at Doncaster and Bassetlaw Teaching Hospital.



The role sits across South Yorkshire Integrated Care Board (SY ICB), Rotherham, Doncaster and South Humber NHS Foundation Trust (RDaSH), City of Doncaster Council and Doncaster and Bassetlaw Teaching Hospitals.



The Care Transfer Hub is centred around Home First, why not home, why not today. We want to reduce the risk of harm to the person by delays in their transfer of care and we want to improve people's outcomes, home is best. The Strategic Transfer of care Hub Manager will provide operational leadership and coordination to enable transfers of care from both acute and community physical health beds and support recovery in the community.This includes the establishment of appropriate governance and assurance, monitoring progress, managing risks and issues and ensuring business readiness for change.

To lead and manage change in relation to the Transfer of Care Hub and related activity and interdependencies

To work autonomously and independently, creating and delivering their own work actions under a broad remit.

Promote new ways of working for admission avoidance and discharge to ensure the right level of care is delivered at the right time and right place.

Ensure effective and timely assurance through programme management and management of interdependencies, risks and issues.

Act as a critical friend to proactively monitor and support the output of the programme and projects within to enable effective delivery of agreed objectives, timescales, quality and cost parameters.

Facilitate and negotiate with stakeholders to ensure timely change according to agreed criteria.

Develop positive relationships and networks, ensuring effective communication and engagement across Place organisations and at all levels including patients, carers, families, Executives, front line staff and others as required.
