Job Description Advanced Clinical Practitioner (ACP) Job Summary The Advanced Clinical Practitioner (ACP) is an experienced, registered healthcare professional who has been assessed as competent in advanced clinical practice through education at Masters level (or equivalent) and mapped against the national Multi-professional Framework for Advanced Clinical Practice. The ACP works autonomously, with the authority to assess, diagnose, treat, prescribe, and manage patients presenting with complex needs, uncertainty, and risk, within the limits of their professional competence. They apply advanced clinical reasoning to support safe decision-making, while ensuring appropriate escalation or referral when necessary. The ACP provides timely, patient-centred care and supports avoidance of unnecessary GP referral, contributing to improved access, continuity, and outcomes for patients. In primary care, the ACP builds long-term relationships with patients, carers, and families, working in partnership to achieve optimum health. The ACP role is underpinned by the four pillars of advanced practice (clinical practice, leadership and management, education, and research). Protected time and structured job planning will ensure the ACP contributes to each pillar, supporting personal development, service transformation, and organisational objectives. Primary Responsibilities The ACP will: - Manage patients presenting with a wide range of acute and chronic medical conditions, exercising independent clinical judgement and decision-making. Undertake extended and supplementary prescribing (V300), adhering to relevant guidance and governance arrangements. Support patients in the safe use of medicines (prescribed or over the counter), conducting reviews and medicines optimisation as required. Contribute to practice and PCN targets (e.g. QOF), complying with local, regional, and national guidance. Identify, manage, and support patients at risk of developing long-term conditions, promoting preventative care. Request, interpret, and act on pathology and diagnostic investigations. Maintain accurate clinical records in line with legislation, professional standards, and practice policies. Support the practice/PCN team in managing clinical emergencies and safeguarding matters, in accordance with policy. Practise within their code of professional conduct and scope of practice, being responsible and accountable for their decisions, actions, and omissions. Demonstrate a critical understanding of their broadened responsibility and autonomy, including working with complexity, uncertainty, and incomplete information. Use advanced communication skills to support shared decision-making, personalised care planning, and positive behaviour change. Exercise professional judgement to assess and manage clinical risk. Act as a professional role model and resource for the wider team. Actively engage in peer review, reflection, and feedback to inform continuous improvement. Involve patients, carers, and communities in the co-design and evaluation of services. Critically assess and address personal learning needs, maintaining a professional development plan that reflects the four pillars of advanced practice. Deliver opportunistic health promotion and patient education as appropriate. Chaperone patients when required. Secondary Responsibilities The ACP may also: - Lead or contribute to service development and pathway redesign at practice or PCN level. Provide structured supervision, mentorship, and educational support for colleagues, students, and trainees. Participate in and lead quality improvement, audit, and research projects, disseminating findings to support evidence-based practice. Support the development of local protocols, policies, and clinical governance frameworks. Engage in structured workplace supervision in line with the Centre for Advancing Practice guidance, ensuring accountability and ongoing capability development. Work towards recognition/credentialing via the Centre for Advancing Practice e-portfolio (supported route) or equivalent, where applicable. Participate in significant event reviews, root cause analyses, and organisational learning activities. Engage with external agencies and professional bodies to promote collaborative working and system-wide improvement.
