Crynodeb
To provide quality, effective pharmacy services to improve patient care including technical and
clinical services, and to assist in education and training of pharmacy and nursing staff.
To promote further development of the pharmacy technician’s role within the Trust.
Key Area 1:Medication Management

Key Area 3: Patient Care
1. Provision of a quality service, demonstrating reliability in completing medication drug histories and skill in non-routine tasks e.g. advising patients/carers on medication issues or handling routine drug queries within HBTT and GPs
2. Development of and participation in Community Mental Health Team duties to include medicines reconciliation, assessment of PODs, attending multi-disciplinary meetings, and liaising with other healthcare professionals to gather relevant information on current medication and compliance status of newly admitted patients within the Home-based Treatment team(HBTT).
3. Assistance in the review and management of the shared care procedures within the Home Based Treatment Team. 4. Contribute to development and implementation of policies and procedures within the department and those impacting across services e.g. Home Based Treatment Medicines Management guideline. 5. Supervise and train new starters within HBTT in the provision of an effective and efficient pharmacy services.
