Crynodeb
We are looking for a Band 6 Personal Health Budget (PHB) Practitioner to support the ICB in managing Personal Health Budgets. The role will focus on developing and reviewing Care and Support Plans, ensuring the effective use of PHB resources to meet individual needs.
Location: Remote (with some face-to-face assessments in Bath, North East Somerset, Swindon & Wiltshire if identified - the ICB will aim to limit these) Hours/Shifts: Monday-Friday, 9am-5pm (full-time)
Contract Type: Temporary, starting ASAP until 31st December 2025
Requirements:Strong experience in completing Care and Support Plans.Knowledge of PHB guidance and processes.Ability to work flexibly and autonomously in a remote environment.Professional registration (e.g. NMP, HCPC)
