We have an opportunity for a permanent Social Care Officer post within our Care Home Assurance and Review Team which focuses on completing statutory reviews and also support quality assurance, monitoring in care homes and improvement of outcomes for adults who live in care homes.
We are looking for an enthusiastic and motivated Social Care Officer who is passionate about improving outcomes for people living in care homes.
You will adopt a person-centered approach that focuses on the adult and their carer/family members whilst undertaking outcome focused assessments and reviews. You will be familiar with Health and Social Care Standards: My Support, my Life and you will complete monitoring of these standards. You will support improvement and contribute to quality assurance in care homes You will participate in duty systems and rotas relevant to CHART and along with colleagues, you will be expected to contribute to the development of innovative ways of working to ensure that adults living in care homes outcomes are improved.

You must have the ability to travel as per service demand.
HNC /SVQ3 in social care or equivalent would be preferred.
You will work 36 hours per week.
If you are the successful candidate, you will be required to gain/maintain PVG scheme membership.
