Working closely with SPINE PCN MDT Team- Provide same-day consultation (face-to-face, telephone, video, or home visits where appropriate) for patients presenting with acute and urgent conditions. Take comprehensive histories, perform focused examinations, and reach working diagnosis for undifferentiated presentations. Initiate and implement treatment, including prescribing medications, wound care, dressings, and minor procedures within scope of practice. Manage common urgent presentations such as: Respiratory infections, sore throats, ear/eye problems. Urinary tract infections, abdominal pain. Musculoskeletal pain, sprains, minor injuries. Exacerbations of asthma/COPD. Fever, rashes, and childhood illnesses. Triage and prioritise patients effectively, escalating to GPs when red-flag symptoms or complex needs are identified. Identify, and manage as appropriate, treatment plans of patients at risk of developing a long-term condition. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency, including initiation of effective emergency care. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side effects and interactions. Support patients to adopt health promotion strategies that encourage patients to live healthily and apply principles of self-care. To request and interpret relevant diagnostic tests (bloods, swabs, ECG, urine dip, etc). Recognise and respond promptly to medical emergencies, arranging urgent hospital admissions when required. Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being. Implement and participate in vaccination and immunisation programmes for both adults and children. To promote and deliver evidence-based care for patients presenting with aural conditions. To work within NMC Code of Conduct and scope of practice as an independent prescriber. To contribute to the development of the practices urgent care pathways and digital triage systems (e.g., Anima). Support colleagues (e.g., junior nurse, HCAs and Physician Associates) in safe urgent care delivery. Engage in clinical governance, significant event analysis, and prescribing audits. To undertake minor ailment clinics or triage. To assist in the supervision and support of health checks. Maintain up-to-date clinical skills and knowledge in acute/urgent care, including safeguarding, BLS/ALS, infection control. Participate in clinical, multi-disciplinary and whole team meetings. Contribute to the delivery of IIF PCN DES indicators were aligned with urgent care. Undertake relevant CPD to support advanced practice and NMC revalidation. Promote equality, diversity, and inclusion in all aspects of care.
