Working autonomously whilst working as part of a multi-disciplinary diabetes team assessing and managing a caseload of diabetes patients maintaining effective communication with patients, carers and professionals to ensure seamless service delivery. Provide expert knowledge of diabetes and work within an agreed job plan to meet diabetes service needs. This should include expert knowledge, understanding and competency in the use of insulin pumps and insulin pump therapy, insulin administration, all diabetes treatments, as well as monitoring devices including continuous blood glucose monitors. Undertake nurse-led diabetes clinics, conducting and interpreting relevant tests and investigations. Work in collaboration with the multi professional diabetes team, supporting medical review clinics, contributing expert, evidence based diabetes knowledge and skills. This should include participating in the joint obstetric/diabetes antenatal clinic to provide diabetes to pregnant women who have pre-existing or gestational diabetes. The consultant led insulin pump clinic and medical review clinics. Maintain boundaries of own practice within agreed guidelines and protocols, use professional judgment in managing complex and unpredictable situations. Address diabetes patient's risk factors and diabetes health promotion needs through comprehensive assessment of individuals who have diabetes to reduce the risk of diabetes related complications. Establish an environment of trust and rapport, enabling patent empowerment in their diabetes care and acting as an advocate for the patient in diabetes treatment decisions and choices Utilise appropriate range of multi-agency and inter-professional resources to ensure that diabetes patients receive appropriate services, making referrals to other professions and services as appropriate. Excellent and effective communication and interpersonal skills to provide information, explanation and reassurance to diabetes patients in a sensitive, empathetic caring manner.
