We have a great opportunity for an experienced Occupational Therapist to join the Disabled Facilities Grant (DFG) team, working part-time 22.5 hours per week. This role focuses on housing adaptations and the progression of Disabled Facilities Grants, supporting residents to live independently and safely in their homes.

About you...

As an Occupational Therapist in the Disabled Facilities Grant team, you will have recent and relevant experience in major housing adaptations.

You will be capable of working creatively with others to develop practical solutions where the home environment affects a person’s independence, function and wellbeing.

You should also be able to assess and match individuals to void properties when their current accommodation is unsuitable and cannot be reasonably adapted.

The Disabled Facilities Grant team is a small, busy and supportive service that works closely with Adult Social Care, housing partners, contractors, and Housing Associations.

We deliver person-centred adaptations that meet long-term needs and ensure the best outcomes for our clients.

As a team, we support each other by working collaboratively to provide a holistic, timely, and cost-effective service that makes a real difference in people’s lives.

Key responsibilities...

Carry out person-centred assessments and work with service users and colleagues to create practical home adaptation plans that promote safety, independence and enable individuals to live safely in their own homes.
Contribute to capacity building across teams by sharing knowledge of major adaptations, legislation, eligibility criteria, and associated processes, while actively participating in multidisciplinary meetings and case discussions where housing needs and adaptations are considered as part of wider care and support planning.
Maintain accurate, up-to-date electronic records, ensuring that assessments, decisions and outcomes are clearly documented and write clear, evidence-based reports and professional recommendations to support DFG applications and specifications for adaptations.
Make timely referrals and signpost individuals to relevant services, community support or other specialists, when needs fall outside the DFG scope or where additional support is beneficial.
Actively engage in supervision, performance discussions, and reflective practice while operating within professional competence, seeking support when needed, and committing to ongoing training and development.
To ensure all practice complies with the policies, procedures and standards of the council, as well as the Health and Care Professions Council (HCPC) Code of Conduct and Standards of Proficiency.
