Specific duties: Clinical: Work independently, without day to day supervision, to assess, diagnose, and manage patients presenting with MSK conditions presenting for the first time to GP practices; Deliver clinics in Primary Care where patients with musculoskeletal conditions can be seen and treated; Make full use of the scope of Advanced Practice, utilising/developing skills relating to independent prescribing, injection therapy and investigation; Make professional judgements and decisions in unpredictable situations, including when provided with incomplete or contradictory information and to take responsibility for making these decisions; Develop integrated and tailored care programmes in partnership with patients, providing a range of first line treatment options including self-management, referral to rehabilitation focussed services and social prescribing; Manage complex caseloads, including working with patients with long term conditions, co-morbidities, psychosocial and mental health needs, referring onwards as required and including social prescribing when appropriate; Communicate effectively with patients, and their carers where applicable, complex and sensitive information regarding diagnoses, pathology, prognosis and treatment choices supporting personalised care; Request and follow up investigations (such as x-rays and blood tests) and referrals to facilitate the diagnosis and choice of treatment regime including, considering the limitations of these investigations, interpreting and acting on results and feedback to aid patients diagnoses and management plans; Ensures care is proactive, preventive in focus and population based, with an emphasis on early intervention; Maintain accurate and up-to-date patient records; Maintain professional knowledge by keeping up-to-date with developments in the field of musculoskeletal conditions/medicine. Organisational: Work as part of a multi-disciplinary team in a patient facing role, using the expert knowledge of movement and function issues, to create stronger links with wider services through clinical leadership, teaching and evaluation; To contribute to developing and reviewing pathways of care for musculoskeletal patients in conjunction with the Community Physiotherapy Service, GPs, PCNs, Secondary Care Consultants, other Health Care Professionals in line with the organisational, regional and national objectives; Provide leadership and support on MSK clinical and service development across the PCN, alongside learning opportunities for the whole multi-disciplinary team within primary care; Develop relationships and a collaborative working approach across the PCN, supporting the integration of pathways in primary care; Encourage collaborative working across the wider health economy and be a key contributor to supporting the development of physiotherapy clinical services across the PCN. Governance and Audit: Actively participate in clinical governance within the PCNs and the member practices; Observe and maintain strict confidentiality with regards to any patient / family / staff / records and information in line with the requirements of the Data Protection Act and Practices policies and procedures; Adhere to and participate with practices in infection control policies and procedures; Maintain professional registration with the Health and Care Professions Council (HCPC); Undertake mandatory training and any other training relevant to the role as required by PCN Practices or OurNet Health Services; Undertake and/or participate in regular audits to monitor standards as required; Promote research and critical evaluation of all clinical practices and ensure that service provision is evidence based. Training and Development: Take part in the training of other staff within the PCN relevant to the role, including lecturing, teaching, training and supporting other members of the multidisciplinary team e.g. Practice Managers, GPs, receptionists. Participate in accredited / validated courses to demonstrate continuing professional development. Be responsible for maintaining own competency by attending in-service training, peer review, individual training and courses. Undertake any other training, which may be relevant to the job role and gain necessary skills to further develop the service.
