Clinical Pharmacy in General Practice is an integral part of transformation to develop a new model of care which addresses our ambition to deliver person-centred, coordinated care across the PCN. The Clinical Pharmacy in General Practice model is supported by the direction of national policy, where there is a need to better utilise the role of pharmacy within primary care to pro-actively help patients stay safe and well and out of hospital as well as helping to reduce the demands on general practice. Clinical Pharmacist play a key role, complementing and working alongside the PCN pharmacy technicians and other members of the PCN multi-disciplinary team. Ideally, the post holder will be a prescriber, or willing to complete training to become a prescriber. You will be responsible be responsible for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially older people, people in care homes, those with multiple co-morbidities (in particular frailty, COPD and asthma) for encouraging the development of better understanding of the principles of medicines optimisation throughout the PCN and practice teams, promoting good practice in line with national requirements, NICE implementation and utilisation of. Action clinical letters and discharge summaries, providing leadership on person centred medicine optimisation and quality improvement. Assist with drug monitoring including high risk drug monitoring, switching medications to agreed and approved protocols, Manage the repeat prescribing reauthorisation process, by reviewing requests for repeat prescriptions & medicines reaching review dates, minimising clinical risk and aiming to reduce wasted medicines. Experience of community pharmacy, Primary or Secondary care would be beneficial to the role, we welcome applications from newly qualified pharmacists. It is a requirement of the PCN to complete the CPPE Pathway, The Clinical Pharmacist team will support you throughout the pathway training, if not already completed.
