The post holder will act as an expert resource, collaborating provide specialist nursing knowledge and expertise on all aspects of inherited bleeding disorders. The post holder will be responsible for the development of a home treatment service, including delivery of haemophilia care in the community, and will have a major role in the future development of the UHCW haemophilia service as it evolves. Facilitate the co-ordination the care of patients throughout the care pathway with the multidisciplinary team and clinical services to ensure the effective management of this group of patients. Demonstrate advanced practice in clinical and holistic assessment, planning, implementation, and evaluation of patients requiring care to ensure that optimum physical and psychological care needs are met. Act as a role model in the delivery of care to Haemophilia and Bleeding Disorder Patients. Including assessment, planning and delivery of care in emergency departments and follow up support demonstrating high standards of holistic care and providing clinical leadership. Highly developed communication skills enabling maintenance of high-quality service and ensuring timely management and resolution of unexpected events and situations. Create a climate in which patients and carers are empowered to make informed choices about their health and treatment options and act as an advocate for the patient/carer. Provide and maintain expert knowledge and practice in clinical skills such as venepuncture, peripheral venous cannulation, and management of central venous access devices. An expert resource for staff, advising on national, local and Trust policy and procedures/guidelines in line with the National Service Specification for Haemophilia and related bleeding disorders, ensuring that clinical governance is embedded in practice. Co-ordinate referrals to Specialist Services, including genetic counselling, carrier detection and Obstetrics. To devise and deliver specialist information related to the condition to patients and carers. This will include teaching patients and their carers to venepuncture, give subcutaneous injections, access central venous lines and ports and administer clotting factors, with the aim to empower them to manage their condition. To record all patient related work to ensure this is commissioned correctly and is accurate and up to date. To manage and maintain the UHCW NHS Trust stocks of Factor Concentrates.
