Responsibilities
Assess clients' needs through interviews and observations.
Develop individualized care plans based on client assessments.
Provide counseling and support to clients and their families.
Facilitate access to community resources and services.
Advocate for clients within the healthcare system and community settings.
Monitor client progress and adjust treatment plans as necessary.
Collaborate with multidisciplinary teams to ensure comprehensive care.
Document all client interactions and maintain accurate case records.
Requirements
Master's degree in Social Work (MSW) from an accredited institution.
Current state licensure as a Social Worker.
Strong knowledge of social work principles and practices.
Excellent communication and interpersonal skills.
Experience in case management and providing direct services to clients.
Ability to work effectively with diverse populations.
Strong organizational skills and attention to detail.
Hours Per Week: 37.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £18.26
