The post-holder will provide general medical services managing patients registered with Tattenham practice and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients The practice has a commitment to excellent patient care, quality improvement and teaching. It is expected that all of our GPs are actively committed to supporting these aims and participate in relevant activities when appropriate. The postholder will also take part in leading on key disease areas (monitoring trends in treatment and prescribing) and assist in developing appropriate healthcare services for a diverse, patient list (practice based commissioning, patient participation and health promotion. Clinical responsibilities: In accordance with the practice timetable, as agreed, the post-holder will carry out a variety of duties including daily triage; surgery consultations; telephone consultations and queries; visiting patients at home; checking and signing repeat prescriptions and dealing with queries; managing patient test results, patient medical reports and examinations (e.g. Insurance Companies) on behalf of the Practice; and paperwork and correspondence in a timely manner. Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation. Recording clear and contemporaneous consultation notes to agreed standards using our clinical system (Emis Web) Issuing acute and repeat prescriptions using electronic prescribing wherever feasible. Reviewing correspondence from hospital clinics and other professionals, and acting on advice as needed Prescribing in accordance with locally agreed or national guidelines. In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care. Teaching and Training Deliver the appropriate parts of educational programmes for medical and nursing students. Supervise Foundation Year doctors or GP Registrars placed at the practice as required Participate in the induction of new staff, students and other attached professionals as necessary. Participate in appraisal, reviews and educational events within the Practice in order to update skills and knowledge. Ensure mandatory training is kept up to date at all times Personal/Professional Development In addition to maintaining continued education through attendance at any courses and/or study days necessary to ensure that professional development requirements are met, the post-holder will participate in any training programme implemented by the practice as part of this employment, such training to include: Participation in an annual individual performance review, annual GP Appraisal including taking responsibility for maintaining a record of own personal and/or professional development; Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work. Organisation and Quality The post-holder will strive to maintain quality within the practice, and will: Recognise and work within own competence and the professional code of conduct for doctors as set out by the General Medical Council. Follow Practice policies, including Security and Information Governance. Complete administrative tasks efficiently and promptly Prioritise, organise and manage own workload in a manner that maintains and promotes quality. Deliver care in accordance with NICE guidelines, evidence based care and local policy Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation. Utilise the audit cycle as a means of evaluating the quality of work, implementing improvements where required. To participate in quality improvement initiatives, including Significant Event Analysis, peer review and the review of patient complaints. To cooperate fully and openly with the investigation of patient complaints (or other investigations), including drafting responses to complaints as appropriate. To support and work towards the achievement of national and local standards (e.g. Quality and Outcomes Framework), and Practice standards. To participate in clinical initiatives and programmes as required. To support the aims and objectives of the Practice and contribute to the ongoing development of the Practice as required. Communication The post-holder should recognise the importance of effective communication within the team and will strive to: Communicate effectively with other team members to ensure the smooth running of the practice Communicate effectively with patients and carers recognising their needs for alternative methods of communication. Recognize peoples needs for alternative methods of communication and respond accordingly. Liaison with members of the Primary Health Care Team, Hospitals, and other agencies (such as local authority, social services and voluntary agencies) in order to assure appropriate care is provided for patients. Demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment. To communicate difficult, often unpleasant or sensitive, messages to patients and families. Act as an advocate when representing patients and colleagues. Participation in Clinical and Practice Development Meetings as appropriate. Teamwork Work as an effective and responsible team member, supporting other members of the team in a flexible and approachable manner. Ensure clear understanding and utilisation of services and referral mechanisms and policies to meet patient need. Contribution to the planning and implementation of services The post-holder will: Apply practice policies, standards and guidance; Discuss with other members of the team how the policies, standards and guidelines will affect own work; Participate in audit where appropriate; Work with the Partners and Management Team to achieve standards of quality, performance standards, budgets and targets without compromising levels of patient healthcare. Contribute towards the development and implementation of new standards, policies and procedures as required Responsibility for Information Resources Review, enter and process data using accurate codes in order to ensure easy and accurate information retrieval for monitoring, financial and audit processes. To understand and follow the requirements of confidentiality (including the Data Protection Act) and the Freedom of Information Act and to refer on any queries as appropriate. To follow Practice policy regarding the use of email and computer tasks; and the use of the internet as the main source of internal and external information. Other responsibilities with the Practice: Maintenance of a tidy and organised work area. Awareness of and compliance with all relevant practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safety, annual QOF Assessment of the Practice. A commitment to life-long learning and audit to ensure evidence-based best practice; Contributing to evaluation/audit and clinical standard setting within the organisation; Attending training, Practice Meetings and events organised by the practice or other agencies, where appropriate. Meet all tight timescales/deadlines for audits and written returns to ensure that the Practice meets quality standards and receives the designated funding (e.g. Quarterly Enhanced Services returns, annual QOF audit etc.) The following information is indicative of current arrangements, but these are subject to change at the discretion of the Practice. The continuing development of the Practice will influence ongoing changes in work patterns, and some flexibility is necessary. Sessions, Rotas etc GPs are expected to contribute to the working of the Practice in whatever way is necessary, and some flexibility is required in hours worked. A part time week will be made up of 6 clinical sessions (including surgeries, prescriptions, visits, clinics, meetings and administration Surgeries contain a mixture of 10-minute face-to-face appointments a mixture of urgent and pre bookable appointments, all GPs are expected to support them in the event of high demand. It is expected that the post-holder will take part in all Practice activities. Attempts will be made when time-tabling the rota for the post-holder to work regular days/sessions each week, but this may vary according to holidays and other requirements of the rota, thus some flexibility is essential. Non-NHS Services The Practice carries out most of the usual additional services for its patients and the post-holder will take part in this. Any income earned in this way will be payable to the Practice. Such services include government reports and examinations, insurance reports, legal reports, medical reports and travel medicine. This job description is subject to periodic review and amendment in consultation with the post-holder in the light of changing circumstances.
