Crynodeb
The LAP team involves supporting people with multidisciplinary support to look at the needs of the resident in a holistic way helping to prevent longer term challenges and admissions to hospital. Your role will involve: Assessments and interventions for residents with physical disabilities, not excluding cognitive decline Working with other professionals to support identified outcomes Holding and managing a case load
