Responsibilities
Conduct assessments of clients' needs and circumstances.
Develop and implement personalized care plans in collaboration with clients and stakeholders.
Monitor and review client progress and adjust care plans as necessary.
Provide advocacy and support to clients navigating services and resources.
Maintain accurate and up-to-date client records in accordance with legal and organizational standards.
Collaborate with multidisciplinary teams to coordinate care and services.
Stay informed about local resources and services available to clients.
Facilitate referrals to external services and community resources to support client needs.
Requirements
A recognized qualification in social work or a related field.
Current registration with the relevant regulatory body.
Previous experience in social care or a related environment, particularly with adults.
Strong communication and interpersonal skills to engage with clients and their families.
Ability to assess and analyze complex situations and develop appropriate interventions.
Knowledge of relevant legislative frameworks and policies related to adult care.
Strong organizational skills and attention to detail in record-keeping and documentation.
Hours Per Week: 36.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £23.86
