An exciting opportunity has arisen for a dynamic Nurse or Nurse(s) to join the amazing Poole Intermediate Care Service. We are a multidisciplinary Team who prevent hospital admissions and secure early hospital discharges.

We deliver a hospital at home service, caring for patients in the comfort of their own homes.

This is a temporary post to cover secondments and maternity leave. Candidates can be considered for Band 5 or Band 6 Nurse roles in the Team.

This role is a minimum of 20 hours per week.

The post will be a great opportunity to develop new skills and experience working in a Hospital at Home Service and may lead to a permanent post in the future.

Applications from candidates that require current Skilled worker sponsorship to work in the UK are unfortunately ineligible to apply for this position.



To demonstrate leadership.

To provide a high standard of holistic assessment and appropriate timely reviews.

To plan specialist care and/or rehabilitation programme, to promote and sustain independence/well-being for the individual in their own homes.

To deliver Nursing/Rehabilitation/care programmes for adults, many of whom have complex clinical and social needs with the aim of improving independence, function and quality of life.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

This is a fixed term role and for those with current NHS service, the role will be offered on a secondment basis only. Please obtain permission to be released on a secondment before submitting an application.
