Post: Diabetes Specialist Nurse Hours of Work: 37.5 hrs per week Reports to: Nurse Manager Key Responsibilities Accountable for own professional actions: specialist for a defined area of diabetes care. Assesses, plans and implements care for patients with diabetes, providing specialist advice and maintaining associate records. To participate in QOF reviews for other clinical indicators such as Hypertension, Chronic Kidney Disease, Rheumatoid Arthritis, Asthma and Chronic Obstructive Pulmonary Disease. This should be with liaison with other HCP for example Nurse Practitioners, Clinical Pharmacists and Health Care Assistants in North Brink team. Co-ordination of these areas should link in with lead GPs and Patient Services Leads. Provide specialist education and training to other staff on all aspects of diabetes management. Planning and Organisational Duties Assesses, develops and implements specialised programmes of care providing specialist advice on all aspects of diabetes management to patients and relatives. Advises medical and nursing staff on appropriate treatment regimes for patients on a daily basis, and is responsible for the continued management of cases in nurse run clinics and home visits. Establish and maintain effective communication with various individuals and groups on potentially stressful topics in a range of situations. Collect, collate and organise data on the health and well being and related needs of people with diabetes and the associated complications drawing the appropriate conclusions and making them available to relevant people. Contributes to the development of specialist evidence based protocols for use within their own area of diabetes care. Raise communities and populations awareness of health and well-being and the actions that can be taken to address issues. Prioritise and monitor workload within own specialist area, recognising change in priorities and implementing corrective actions where necessary. Contributes to the planning and delivery of specialist education for health professionals. Communications and Key Working Relationships Accept direct referrals from other health care professionals and through the utilisation of relevant diagnostic investigations makes a diagnosis and generates treatment options. Analyse and rigorously review all aspects of the patients programme of care interpreting information and using knowledge and judgement to provide highly specialist advice to recommend new course of action where necessary. Skilled in the assessment of the special needs of the elderly and their limitations which have to be considered when producing individualised treatment regimes (eg cognitive function, manual dexterity) Explain clearly to patients/carers the benefits and risks of different interventions, the alternative approaches available and the role of individual members of the team. Communicates empathetically comprehensive and detailed sensitive information to patients and carers regarding both the diagnosis and the development of long-term life threatening complications. Utilise highly developed physical skills where accuracy is important, eg teaching home blood glucose monitoring. Support patients/carers encouraging them to promote their own health and well-being and to express their interests and concerns. Identify potential communication difficulties, relevant contextual factors and the risk of not communicating information at an appropriate time. (eg providing individualised patient education at level and appropriate timing for them, taking into account barriers to communication such as language, physical disability and environment.) Compliance with the Health & Safety at Work Act 1974 the post holder is required to fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards, and a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions. Responsibility for Human Resources Maintain own professional development whilst evaluating own specialist knowledge and practice through a system of appraisal and personal development planning. Undertake annual mandatory training updates and other relevant courses in line with Practice policies. Act as a role model providing mentorship and/or clinical supervision to other staff/learners supporting them in applying theory to practice. Accountable for own professional actions undertaking all duties having regard to the NMC Code of Professional Conduct for the nurse, midwife and health visitor and their professional contribution to Clinical Governance. Responsibility for Teaching Identify the wider benefits, and influence and prioritise the development of knowledge, ideas and work practice in self and others to meet the demands of the job Establish with others goals and methods for developing knowledge, ideas and work practice within diabetes care (e.g. teaching sessions, education forums) Take an active part in sharing achievements and developing knowledge, ideas and work practice (organising and contributing to departmental, trust, local and national education forums) Promote awareness for national policies and other initiatives related to diabetes, eg NSF, NICE Guidelines and local Guidelines. Communication Provide and receive highly complex, sensitive or contentious information and occasionally deal with situations which are hostile or emotive Use developed communication, negotiation, and conflict management skills which require empathy, reassurance and persuasive skills to overcome barriers to acceptance. Make operational judgements, manage conflicting views and reconcile differences in professional opinion. Build and communicate effective working relationships with a wide array of statutory and voluntary organisations for the benefit of patient care and facilitate good working relationships. Manage self and others Participate in an annual individual performance review, taking responsibility for maintaining a record of own personal and professional development. Manage own time, workload and resources effectively Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance. Assess own performance and take accountability for own actions, either directly or under supervision. Undertake professional development as required by the role. Encourage and motivate others to be forward thinking in ideas that address the health needs of our patients and be innovative whilst managing and developing new ways of working. Promote effective communication and relationships within the team. Engage in clinical supervision with peers and expect clinical supervision for own development Participate positively and actively in clinical and practice meetings Develop own knowledge and clinical practice to meet own objectives and changes in service Be involved in mentoring Nurses undertaking extended roles in chronic disease management and minor illness Lead on standards setting and promote best practice Manage and use information Alert other team members to issues of quality and risk. Make effective use of the clinical system by ensuring consistent and accurate data entry and use of Read Codes Facilitate the use of up to date information technology in order to collate accurate and timely information as and when required by the Practice. Assist in the development of practice protocols for the surgery and clinical protocols for the clinical team Confidentiality In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters.
