An exciting opportunity has arisen to become a B7 community palliative care nurse team (CPCT) as a Specialist Palliative care nurse as a rapid response / floater nurse supporting patient across South Derbyshire.

The team provides patient centred, evidence-based specialist service for patients and their carers with palliative and and end of life care needs, working where required across south Derbyshire.

In addition to direct clinical role the post includes leadership, managerial, educational and audit/research responsibilities.

The post holder will be expected to have flexible approach to working, which includes weekends and bank holidays.
• To be flexible and adaptive to support the needs of the service.
• To be adaptable to a range of situations that can arise and be able to re prioritise as patients needs change.
• To be able to deal with challenging situations that can be highly emotive and have exceptional communication skills.
• To take overall responsibility for the care of patients with a palliative diagnosis, requiring specialist nursing care.
• To offer the patient and those important to them the skills of a CNS in symptom control, social, emotional and spiritual care.
• As part of the team it is expected that you will be able to participate, implement and assess symptom management.
• To work from a multidisciplinary approach to care
• To participate in maintaining and developing a quality service.
• To participate in the provision of advice and training across functional boundaries/health community as required.
• Ensure the delivery of agreed professional standards of care within their specialist area.
• To participate in research and audit within their specialist area and support with the dissemination of its findings.
• Maintain their own professional development required by the trust and that of the NMC
