The Hounslow community rehabilitation service is looking for a motivated, dynamic Occupational Therapist to join the Community Rehabilitation Therapy Team. You will join an established team of Occupational Therapists, Physiotherapists and Rehabilitation Assistants working in an integrated service to provide client centred rehabilitation programme for Hounslow residents.

The Community Recovery Service is an integrated health and social care service for adults identified with rehabilitation, reablement and recovery needs that can be met in the community. The service will support independence by providing rehabilitation and reablement after an acute illness, injury or change in life circumstances. It aims to restore, maximise or prevent deterioration in physical, psychological and social functioning through episodes of rehabilitation and reablement.

The Community Recovery Service also provides the Hounslow Home First Pathway and works with the Integrated Discharge Hub for West Middlesex University Hospital. Both services are run 7 days per week.

Provide high quality and holistic Occupational Therapy assessment and treatment to adults referred to the Community Recovery Service.

Perform skilled occupational therapy assessment of patients with a range of conditions, who may have complex physical and psychological conditions.

Provide evidence-based treatment programmes, guided by patient orientated goal setting whilst liaising with the multidisciplinary team.

Be responsible for organising and planning own caseload to meet service and patient priorities. Readjusting plans as situations change/arise.

Supervise, educate and assess the performance of rehabilitation and therapy assistants, in line with RCOT standards of clinical practice for Occupational Therapy assistants
