Crynodeb
Work as a Specialist Speech and Language Therapist within the Early Child Development Pathway as part of a multi-disciplinary team, and to provide support to Speech and Language Therapists working across all other integrated pathways as required. Screen and triage referrals and ensure a strengths-based approach to assess, plan, implement and evaluate interventions to support the complex packages of care for children with complex additional needs in the community. Contribute to risk assessment and care planning in collaboration with children, young people and their carers. Provide specialist SLT assessment, advice and interventions to young children and their families and to hold a caseload of children with complex needs. Provide case management of an identified caseload of children and young people with highly complex long-term conditions and complex needs by utilising and promoting evidence-based care.
