Job brief
Are you passionate about supporting adults, predominantly aged 18-25 years with a learning disability and/or autism and their families?
Do you have experience of working in a supporting role, gained through settings such as social care, health, the community, or further education?
Can you demonstrate strong communication skills, safeguarding awareness, and experience of working in a multi-disciplinary team?
Would you be interested in holding a small caseload of individuals, working intensively with them to prevent crisis and /or hospital admission?

We can offer you a comprehensive training package, support and supervision and no two days that are the same!
Hours-37.5 hrs per week Initial 1 year contract with further extension subject to funding
Salary- £25,500 starting salary
Salary banding £25,500-£27,000.

Key Tasks
Person Centred Planning
Maintain a caseload of adults up to the age of 25 years, who are currently on the Dynamic Support Register (DSR).
Work with the adult, and their family as appropriate, to develop a personalised plan based around their needs. Identify and agree outcomes and goals and deliver this plan flexibly.
Facilitate the provision of any identified support and/or interventions as necessary to stabilise situations and prevent inpatient admission.
Support young people to develop their own solutions, led by their needs, wants and aspirations.
Use creative and accessible techniques to communicate with the person you are working with, depending on their various communication needs e.g., pictures or role play.
Support and contribute to successful discharge planning, ensuring the voice of the adult is incorporated at all times.
Advocacy
Actively listen and accurately capture the individuals’ views and preferences and convey these to other services as needed.
Communicate clearly with the individual and their family, where appropriate, to help them understand and navigate systems.
Represent and champion the individual and their wishes, views and preferences in multi-Disciplinary meetings.
Advocate for individuals to ensure the necessary support is in place to enable them to remain within the community.
Promote a culture which prioritises keeping a young person within the home/community by effectively communicating with a range of services including staff at different levels of seniority.
Identify changes that would lead to better outcomes for those with autism and/or learning disabilities.
Ensure action points from meetings are completed in a timely manner by yourself and any other services responsible.
Signposting/Multi Agency Working
Identify, guide, and refer individuals to other services where appropriate
Provide support to access the right help at the right time across complex systems, particularly the transition between children and adult’s services.
Build relationships and support coordination across the health, education, and social care system.
Maintain an up-to-date knowledge of the local offer and keep up to date with changes to provision to ensure effective signposting and referrals
Support to navigate statutory and non-statutory services within the community.
Safeguarding
Maintain accurate and up-to-date knowledge of relevant legislation and frameworks including Mental Health Act, Mental Capacity Act, Deprivation of Liberty Safeguards, 117 After Care Procedures, Human Rights Act.
Actively safeguard children, young people, and adults within your day-to-day role, ensuring that any concerns are escalated as appropriate.
Attend all mandatory safeguarding training and ensure Daisy Chain’s policies and procedures are always adhered to.
Record Keeping
Maintain clear, accurate records of work with individuals, in accordance with Daisy Chain’s record keeping procedures.
Keep track of progress to gauge ongoing support requirements
Undertake administrative tasks necessary for the efficient and effective delivery of the key responsibilities in this role.
Additional responsibilities:
Comply with the Charities’ Health and Safety policy in all employment practices
Work in ways, which promote equality of opportunity, participation, and responsibility
Maintain the integrity of the Daisy Chain brand in all internal and external communications
Maintain a proactive approach to your role and responsibilities along with a commitment to contribute and maintain the positive working ethos within the Charity
Ensure attendance at staff meetings.
Work in a way that contributes to the continuous improvement of the Charity’s services and embraces the organisations quality initiatives
