We have an exciting new opportunity to join Tameside as Transitional Care nurse. In this role you will facilitate the implementation and evaluation of care pathways within neonatal Transitional Care (TC) on the maternity ward, working closely with the maternity and neonatal teams. The focus of this role and Transitional Care is to keep parents and babies together while providing enhanced care, preventing avoidable admissions to the neonatal unit.
It will involve working with the Inpatient Maternity Matron, Maternity Managers, and Neonatal Intensive Care Unit (NICU) Manager strategically and operationally to meet the British Association of Perinatal Medicine (BAPM) recommendations and Framework for Neonatal Transitional Care in accordance with guidelines. Successful implementation of TC requires collaborative working between midwifery, neonatal, and the wider multidisciplinary teams
The post holder will strive for consistent delivery of high-quality care and be responsible for providing care for newborns who require TC, for example, intravenous (IV) therapy, naso-gastric (NG) feeds, and phototherapy.
You will act as a clinical leader and expert practitioner, liaising and working collaboratively with the multi-disciplinary teams and external agencies as required in the provision of optimal care of babies and their families. In this role you will be based on the postnatal ward, and maintain strong links with NICU and the neonatal team.
