To promote, encourage and develop integration between Community and inpatient services. To demonstrate knowledge and skills within the defined area of expertise; utilising expert clinical knowledge and nursing skills to provide assessment, intervention and clinical guidance as appropriate. To create, develop and maintain professional supportive relationships with all members of staff with other professionals and agencies to enhance recovery. Working with MDTs in management of care and responding to changes in service users health. To recognise potential crisis situations and act responsively and responsibly, to resolve the situation. Understanding the range of risks potentially faced by the service users and working in partnership with others to identify and assess the nature, location and seriousness of the particular risks and to advise on appropriate action. Ensure effective planning for operational delivery of the service over the next one to two years, working with the Associate Director of Urgent Care, Head of Service and wider Care Group Management, adjusting plans in response to internal and external factors. Ensure that services are safe and effective and delivered in accordance with evidenced based standards and recognised good practice e.g. NICE guidance. Ensure that all statutory and professional responsibilities of the NMC Codes of Conduct are met alongside other professional related bodies. Establish effective clinical governance systems and processes e.g. facilitating clinical audits. Implement recommendations from clinical audits with support from the Clinical Care Director, Clinical Lead and Professional Leads. Develop local policies and standard operating procedures across the service to support service delivery. Provide immediate advice and direction to team members in relation to complex service issues, analysing and interpreting the various options available. Provide managerial, technical and where appropriate clinical advice to other disciplines and agencies in relation to clinical service. Ensure that national and local targets and key performance indicators are met in accordance with service specifications / contracts, taking appropriate action where indicators are not being met. Lead in the management of risk related to the delivery of clinical services; identifying and monitoring risk, taking action where necessary to mitigate risk and escalating risks, as appropriate. Ensure adverse events / serious incidents are appropriately investigated and appropriate service changes are facilities to avoid further incidents. Ensure systems are in place to respond promptly and appropriately, in an open manner, to complaints and queries concerning the service. Investigate complaints, meet and respond to complaints and draft complaint responses as required. Provide advice, information and support in relation to the Council and NHS Complaints procedures. Implement associated actions and recommendations. Preparing reports and analysing data to evidence service performance and clinical outcomes, as requested by the Associate Director of Urgent Care. Provide cross service management cover and deputise, as appropriate providing cover for annual leave and other absences. Participate in the Out of Hours Management on-call rota (as necessary and relevant to clinical area), provide advice, guidance and direction to staff working out of hours. Manage a delegated budget across areas of responsibility, maintaining financial balance and ensuring effective and efficient use of resources. Manage the purchase of resources and equipment required for the delivery of services. Act as signatory, within authorised limits, in accordance with Standing Financial Instructions.
