Crynodeb
Run clinics to conduct medication reviews and resolve patient medication queries (mainly via telephone consultations, with a blend of remote and on-site working). Reconcile medicines following hospital discharge or transfer of care. Manage repeat prescription requests and queries within scope of practice. Implement changes in line with MHRA alerts, product withdrawals, shortages, and guidance. Signpost patients to appropriate services or healthcare professionals where necessary. Support QOF, IIF and quality improvement projects. Undertake audits and implement recommendations. Maintain accurate clinical records, using templates and coding appropriately. Identify clinical risks and contribute to developing safer, more effective systems. Maintain clinical competence through CPD and professional development.
