Crynodeb
To uphold and actively support the Trusts smoke-free status Be aware of, and contribute to, project targets. Where appropriate to refer into specialist-led clinic-based support Shadowing TDT specialists, develop expertise in effective treatment programmes for people with a range of smoking related health conditions, to enable managing a small caseload To utilise a range of different approaches to raise awareness of tobacco dependence services and to use all possible opportunities to generate referrals for patients. To support the development of seamless care pathways and referral systems between professionals in acute settings, midwifery, community health services and primary care. To support the integration of the tobacco dependence service into broader health development initiatives. To contribute to the ongoing development, delivery, monitoring and evaluation of the COSTED intervention. To be responsible for managing and monitoring ordering of vapes and liquids, ensuring correct data entry and clear communication.
