Summary
As Service Team Manager for the Hospital Discharge Team, you will lead a multidisciplinary team working across acute settings to ensure safe and person-centred discharges. You will: Provide visible, supportive leadership to staff, promoting resilience and reflective practice. Coordinate effectively across health and social care partners to advocate for individuals rights and needs. Ensure compliance with statutory duties including the Care Act, Mental Capacity Act, Continuing Health Care, and safeguarding. Champion strengths-based and person-centred practice in a fast-paced, multidisciplinary environment. Contribute to RBWMs strategic priorities: building inclusive, healthy communities, promoting prevention and choice, and delivering compassionate, high-quality services.
