We have an exciting opportunity for a Service Team Manager to lead our Hospital Discharge Team!

We are recruiting a knowledgeable and compassionate Service Team Manager to lead our Hospital Discharge Team at the Royal Borough of Windsor & Maidenhead. This important role is central to delivering our Social Care Futures vision and ensuring that hospital discharges are safe, timely, and person-centred.

This is a full-time, permanent role with hybrid working, and a salary of £53,296 - £58,286 per annum.

About the role
As Service Team Manager for the Hospital Discharge Team, you will lead a multidisciplinary team working across acute settings to ensure safe and person-centred discharges.

You will:
Provide visible, supportive leadership to staff, promoting resilience and reflective practice.
Coordinate effectively across health and social care partners to advocate for individuals’ rights and needs.
Ensure compliance with statutory duties including the Care Act, Mental Capacity Act, Continuing Health Care, and safeguarding.
Champion strengths-based and person-centred practice in a fast-paced, multidisciplinary environment.
Contribute to RBWM’s strategic priorities: building inclusive, healthy communities, promoting prevention and choice, and delivering compassionate, high-quality services.
