About you...
You will be part of a well-established, multi-professional statutory team, carrying out comprehensive assessments for adults with disabilities. Your work will involve supporting individuals with a wide range of needs, including those with learning disabilities, physical disabilities, dementia and older adults. A key aspect of your role will be enabling people to maintain their independence and quality of life within their communities, working closely with partners in the voluntary and community sector.
You will gain an invaluable breadth of experience across Adult Social Care and will be supported to develop your knowledge and confidence. Rotational posts across the teams will be considered.
We are looking for an enthusiastic, confident and reliable person who is passionate about strength based practice, supporting residents of Bracknell Forest and applicants who have had experience of manual handling situations as a qualified Occupational Therapist.
About the team...
In Bracknell we are passionate about promoting independence and wellbeing through new ways of working and forward thinking interventions. We want to reduce dependence, prevent deterioration and delay reliance on traditional services.
Although we work in three distinct teams, we are one service and we work together closely which leads to good relationships and opportunities for learning from each other. Our offices are in Market Street, Bracknell and we work flexibly from home and around the borough. You will receive regular supervision and attend MDT meetings and you will hold a varied caseload.
Key responsibilities...
To work with people to coordinate the identification of their needs and undertake comprehensive assessments of adults with disabilities. To work with those with diverse or complex presentations, in a way that maximises independence, choice and control, that are Care Act compliant and cost effective.
Responsible for own caseload, working collaboratively with those in the People Directorate.
To be an autonomous practitioner and accountable for all aspects of your work, including management of individuals in your care and your own supervision and appraisal.
To assess for, plan and provide interventions around manual handling, seating and small aids/ adaptations.
Participate in safeguarding assessments and planning when required.

Interview date...
The interviews will be held in week commencing 20th October 2025.
Interviews will be conducted in person, providing candidates with the chance to personally meet the team and experience our work environment.
