Job Title:Band 6 Care Coordinator Trust: South London & Maudsley NHS Foundation Trust Location: 63-65 Marina House, Denmark Hill, SE5 8RS Ward: Assertive Outreach Team Rehab 1 Branch Pay Rates: Band 6, £21.96 per hour Shift Patterns: Mon-Fri 9am-5pm What you can add: Support service users recovery goals and independence, including those with physical health needs, by collaborating with local services and primary care teams. Provide clinical leadership and professional support to junior staff, acting as a role model and supporting their development through supervision. Build and maintain strong working relationships with community, inpatient, and external partner services to promote seamless care transitions. Understand and navigate placement processes and discharge pathways to reduce delays and improve patient outcomes. Maintain accurate, up-to-date clinical records, including risk assessments and contingency planning, in line with Trust policy. In addition to experience, all Qualified roles will be required to provide evidence of: Relevant Qualifications Registration with the relevant Registration Body if applicable.
